When to keep your child home: 

Please keep your child home if they exhibit any of the symptoms listed below. If symptoms are not improving or seem to be worsening please contact your child’s healthcare provider.

  • Fever of 100.0 or greater- may not return until the fever is below 100.0 without the use of Acetaminophen or Ibuprofen for 24 hours

  • Cough that makes a child feel uncomfortable or disrupts the class or prevents the child from sleeping at night

  • Sore throat that is not due to allergies or dry air

  • Honey-crusted sores around the nose or mouth

  • Any unexplained rash

  • Red, runny eyes that distract the child from learning

  • Large amount of discolored nasal discharge, especially if accompanied by facial pain or headache

  • Severe ear pain or drainage from the ear

  • Severe headache, especially if accompanied by fever

  • Loss of taste or smell

  • Nausea, vomiting, diarrhea- may not return until 24 hours after the last vomiting or diarrhea episode

  • Any condition that you think may be serious or contagious to others

     

 Remember to call the attendance office (716-736-7232) if your child will not be in school.

Screenings:

The school nurse, in accordance with the requirements established by the NYS Education Department and school policy, conduct the following screenings annually:

  • Vision

  • Hearing

  • Scoliosis

  • Height and Weight

  • Health Appraisals

The results of these screenings are entered into the student’s cumulative health record. A letter will be sent home if your student needs follow-up with your healthcare provider. For vision screenings only, parents will be notified of all results.  Please call the school’s Health Office if you have any questions or concerns.

*If you do not want your child to be screened at school, please send a written note to the Health Office.

BMI Opt Out Letter for the 2025-2026 School Year: (English)

If you do not wish to have your child’s weight status group information included as part of the Health Department’s survey this coming school year, please print and sign the BMI opt out 2023-2024 School Year form.

Required NY State School Health Examination Form:

NY State Education Department requires a physical exam for new entrants and students in Grades Pre-K or K, 1, 3, 5, 7, 9 & 11. Please have your doctor complete this form and return it to the school nurse. If your primary care provider is located in NY state they may have their own electronic version of this form but if you go to a provider outside of NY state you must take this form with you for the provider to complete.

Dental Certificate:

New York State law recommends that school districts request a dental certificate for each newly enrolled student and/or when entering Pre-K or K, 1st, 3rd, 5th, 7th, 9th, and 11th grade . Print this sample Dental Health Certificate and forward it to your child's dentist for completion. The completed certificate should be returned to the Health Office.

Medical Release Form:

Your healthcare provider will require the Release Form to share Protected Medical Information with the school district.

Medication Authorization Form:

If you wish for medication to be administered to your child during the school day, this Authorization Form must be printed, completed (by both parent AND physician) and returned to the Health Office.
